We at Servant’s Heart Home Health Services are determined to be the leader in innovative, quality home health care. We serve the communities of Carroll, Cass, Clinton, Fulton, Howard, Miami, Pulaski, Tippecanoe, Tipton, Wabash, and White County in Indiana.

We currently offer the following services:

Skilled Nursing Services

Registered Nurses and Licensed Practical Nurses will furnish home visits according to a plan of care and as ordered by a physician.

Services may include:

Medication management (click for more information)
Home blood draws for lab tests
Dressing changes
Patient teaching (i.e.: a newly diagnosed diabetes patient and administering insulin injections)
Monitoring/evaluating changes in physical condition
Case management
Other skilled care services

Referrals & requests for these services AND MORE are accepted from family members, patients, hospitals, nursing homes, physician offices, clinics and community agencies.

Home Health Services

Patients will be provided with home health aides to assist with personal care needs.  These services will be provided according to a plan of care developed and supervised by the Registered Nurse.

Services may include:

Bed, tub, or shower bath
Shaving
Hair care
Linen changes
Light Cleaning
Promoting comfort for the patient
Assistance with ambulation, toileting
Laundry
Various other Home Health Aide services
